Chronic Disease Management & EPC Clients

The Chronic Disease Management Plan, also referred to as the Enhanced Primary Care (EPC) program, is a referral provided by your General Practitioner (GP) that allows you to access physiotherapy services as part of your ongoing care for chronic conditions. This program is designed to help individuals with long-term health issues such as arthritis, back pain, or neurological conditions, offering them the support they need to manage their symptoms and improve their quality of life.

Under the EPC program, you are eligible for up to 5 physiotherapy sessions per calendar year. These sessions are covered by Medicare, helping to reduce the cost of your treatment. However, there may be additional out-of-pocket expenses depending on the services provided, which will be discussed with you during your appointment.

To access this program, simply ask your GP for a referral. Once you have it, you can book an appointment at Physio Den, and our experienced physiotherapists will work with you to develop a treatment plan tailored to your specific needs.
